Case Management

The Case Management staff assist patients and their families in making plans for continued care after discharge from the hospital. They help patients connect to the appropriate resources to meet their post-hospitalization needs. This may involve home health care assistance, rehabilitation services, skilled nursing care or, when appropriate, referral help for long term care.
